ERP and IFS Integration for Anxiety and OCD Treatment

When Anxiety Feels Like It’s Running the Show

Anxiety and OCD can be exhausting.

You may find yourself constantly analyzing, checking, avoiding, researching, reassuring, preparing, or replaying conversations in your mind. Even when you know a fear doesn’t fully make sense, it can still feel impossible to ignore.

Many people come to therapy believing they need to get rid of anxiety before they can start living the life they want. Others feel frustrated because they’ve spent years trying to think their way out of fear, only to find themselves trapped in the same cycles.

After more than 20 years of working with children, teens, adults, couples, parents, and families, we’ve found that lasting change often begins with a different question:

What if anxiety isn’t the enemy?

At Bedrock Therapies, we specialize in humans. Not diagnoses, labels, or symptom lists. We work with real people who are trying to understand themselves and create meaningful change. For many clients struggling with anxiety or OCD, that work involves integrating two powerful approaches: Exposure and Response Prevention (ERP) and Internal Family Systems (IFS).

Through the PAUSE framework, we help clients develop a different relationship with fear—one built on curiosity, self-compassion, and courage rather than endless fighting. The goal is not feeling comfortable. The goal is becoming free.

Why Fighting Anxiety Often Makes It Stronger

One of the hardest realities about anxiety is that the things we do to feel better in the moment often keep anxiety alive in the long run.

Avoidance can temporarily reduce discomfort.

Reassurance seeking can provide short-term relief.

Compulsions can create a brief sense of certainty.

Checking, researching, reviewing, and controlling can make us feel safer for a few moments.

Unfortunately, these strategies teach the nervous system that the fear was dangerous in the first place.

Over time, anxiety grows stronger rather than weaker.

Many clients report feeling trapped between two painful options: giving in to anxiety or forcing themselves to push through it.

There is another path.

What ERP Brings to Anxiety and OCD Treatment

Exposure and Response Prevention, often called ERP, is considered the gold standard treatment for OCD and many anxiety-related conditions.

ERP helps people gradually face situations, thoughts, sensations, or uncertainties that trigger fear while reducing the compulsive responses designed to make that fear go away.

The goal is not to eliminate anxiety.

The goal is to develop a new relationship with anxiety.

ERP helps clients learn:

  • Uncertainty can be tolerated.
  • Anxiety naturally rises and falls.
  • Fear does not always require action.
  • Compulsions are not the same as safety.
  • Confidence grows through experience, not certainty.

Over time, people learn they are capable of handling far more discomfort than anxiety initially suggests.

What IFS Adds to the Process

While ERP helps people change their relationship with fear, Internal Family Systems helps people understand the parts carrying that fear.

IFS begins with a simple idea:

Every behavior makes sense when we understand what it is trying to protect.

The perfectionistic part that constantly checks may be trying to prevent mistakes.

The worried part may be trying to keep you safe.

The obsessive part may be trying to create certainty in an uncertain world.

The avoidant part may be trying to protect you from pain.

Rather than battling these parts, IFS invites curiosity.

In our experience, people often feel tremendous relief when they realize their anxious parts are not broken. They are protective.

Many clients report that self-criticism begins to soften when they understand that these parts are working incredibly hard to help, even when their strategies are no longer serving them.

Why We Access Self-Energy Before Exposure Work

This is where ERP and IFS become especially powerful together.

At Bedrock Therapies, we often use the PAUSE framework to prepare for exposure work:

P — Pause and Notice

What thoughts, sensations, emotions, and urges are present?

A — Acknowledge with Compassion

Can we meet these experiences with curiosity instead of judgment?

U — Understand Protective Patterns

What part of you is trying to help right now?

S — Step Into Self-Energy

Can you access the calm, grounded, curious part of yourself that can lead with confidence rather than fear?

E — Experiment With New Responses

Can we practice a different response instead of following the old anxiety pattern?

When clients access Self-energy before an exposure, the work often feels less like a battle and more like a choice.

Instead of forcing anxious parts into submission, we help clients build enough internal leadership to move forward while honoring the concerns those parts carry.

What ERP and IFS Can Look Like Outdoors

Many of our clients choose outdoor therapy because nature naturally supports the first steps of PAUSE.

In my experience working outdoors, it is often easier for people to slow down, notice what is happening inside, and access curiosity before jumping into problem solving.

Outdoor ERP may include:

  • Touching a gate, bench, or trail marker that feels “contaminated”
  • Walking through uncertainty without seeking reassurance
  • Allowing intrusive thoughts to be present without neutralizing them
  • Practicing response prevention while remaining connected to the present moment

Many clients report that ERP feels less like a harsh test and more like a shared experiment when practiced outdoors.

Nature creates space.

The rhythm of walking creates movement.

The presence of trees, wind, birds, and open sky often reduces the pressure people feel to perform therapy correctly.

Outdoor work also provides real-life opportunities to practice uncertainty tolerance. Unexpected trail users, changing weather, wildlife encounters, and other unpredictable moments become opportunities to build flexibility rather than control.

Anxiety Treatment Is More Than Symptom Reduction

While reducing symptoms is important, our goal is not simply to help you feel less anxious.

Our goal is to help you live more fully.

That might mean:

  • Spending less time in compulsions
  • Building stronger relationships
  • Returning to activities anxiety has restricted
  • Taking meaningful risks
  • Increasing self-trust
  • Developing a more compassionate relationship with yourself

When anxiety is no longer running the show, more energy becomes available for the life you actually want to live.

Supporting Children, Teens, Adults, Couples, and Families

Anxiety and OCD rarely affect just one part of life.

Children may struggle with fears, rituals, or avoidance.

Teens may battle perfectionism, social anxiety, or intrusive thoughts.

Adults often find anxiety impacting relationships, parenting, work, and overall well-being.

Families and couples frequently develop patterns organized around anxiety without even realizing it.

Because we specialize in humans, we tailor ERP, IFS, mindfulness, TEAM-CBT, and other evidence-based approaches to fit the person sitting in front of us rather than forcing everyone into the same model.
